البروتينات هي لبنات بناء الجسم building blocks في كل أجزائه من عضلات و عظام و جلد و أظافر و شعر, كما أن بروتينات الدم تقوم بعدة وظائف منها حماية الجسم من العدوى والمساعدة في تجلط الدم والمحافظة على كمية السوائل الموجودة بالدورة الدموية . وعندما يمر الدم بالكليتين للشخص السليم فإنه يترك عوادم الجسم و فضلاته waste products ويحتفظ بما يحتاجه الجسم من مواد ومنها البروتينات ومعظم جزيئات البروتينات يكون حجمها كبير ولا تستطيع المرور من خلال مرشحات الكلى kidneys' filters إلى البول إلا في حالة وجود تلف damage بالكلى

والبروتين الرئيسي main protein الذي يمكن أن يكون في هذه الحالات متواجد بالبول هو الألبومين albumin, وتستطيع البروتينات أن تهرب من الدم إلى البول في حالة وجود تلف بمرشحات الكلى والتي تسمى كبيبات الكلى glomeruli.

وقد يحدث نزول البروتين بالبول نتيجة عدم إعادة امتصاص البروتين النازل بالبول من القنوات البولية , حيث أنه في الشخص السليم تعيد هذه القنوات امتصاص البروتين بعد مروه من كبيبات الكلى بكميات قليلة .

والبعض من العلماء يعتبر أن نزول البروتين بالبول سام للكلى nephrotoxic وهناك إجماع في الرأي أن التقليل من البروتين المفقود بالبول من خلال العلاج يعتبر ضروري لحماية الكلى renoprotective . و فقدان البروتين بالبول Proteinuria يصف حالة يكون فيها البروتين موجود بكمية غير طبيعية بالبول , ويطلق هذا اللفظ عادة عندما تشير تحاليل البول إلى وجود الألبومين albumin تحديدا .

ومن أهم وظائف الألبومين الاحتفاظ بالسوائل بالدم , ويعتبر العلماء هذه المادة بمثابة قطعة الإسفنج التي تجمع الماء الزائد عن المعدل الطبيعي بأنسجة الجسم وأمراض متعددة مثل السكر diabetes و ارتفاع ضغط الدم hypertension وأمراض بالكلى قد تسبب التهاب كبيبات الكلى glomerulonephritis أو ببساطة التهاب الكلى nephritis وبالتالي تؤدى إلى هروب البروتين من الدم إلى البول proteinuria . وتظهر الأبحاث أن كمية البروتينات الموجودة بالبول ونوعها (سواء كانت ألبومين فقط أو أنواع أخرى من البروتين) هي مؤشر قوى لمدى وجود تلف بالكلى وأيضا مؤشر قوى لمدى تقدم المرض في اتجاه حدوث فشل بالكلى renal failure .

ويعتبر العلماء أن فقدان البروتين بالبول مؤشر يدل على حدوث ضرر لبطانة الأوعية الدموية marker of endothelial cell injury وهذا يزيد من مخاطر الإصابة بأمراض القلب والأوعية الدموية risk factor for cardiovascular disease .

أسباب فقدان البروتين في البول

من أهم الأسباب :

الأمراض التي تصيب نسيج الكلى مثل التي تسبب متلازمة الكلى Nephrotic syndrome .

مقدمات تسمم الحمل Pre-eclampsia وتسمم الحمل Eclampsia .

بعض الأمراض المناعية مثل الذئبة الحمراء الجهازية .

يعتبر مرض السكر من الأسباب الرئيسية يليه ارتفاع ضغط الدم .

ممارسة الرياضة العنيفة Strenuous exercise .

التعرض لضغوط Stress .

الوقوف لفترات ممتدة Benign Orthostatic (postural) proteinuria .

أعراض وعلامات فقدان البروتين في البول

وجود كمية كبيرة من البروتين بالبول يجعل البول ذو رغوة foamy .

تجمع السوائل الزائدة بأنسجة الجسم ( وذلك حيث أن نقص البروتين بالدم بسبب فقدانه بالبول لم يعد يجعله كقطعة الإسفنج التي تمسك بالسوائل الزائدة عن أنسجة الجسم) فيحدث تورم باليدين و القدمين و الوجه و البطن .

والأعراض السابقة تكون عند فقدان كمية كبيرة من البروتين ولكن قد يحدث فقدان للبروتين دون أعراض ظاهرة , ولذلك تكون التحاليل هي المؤشر لوجود البروتين بالبول من عدم وجوده كما أنها أيضا تحدد الكمية الموجودة .

التحاليل والفحوص لفقدان البروتين في البول

يتم التحليل للكشف عن البروتين بالبول باستعمال شرائط يتغير لونها عند غمسها بالبول . و تستطيع معظم أنواع هذه الأشرطة قياس البروتين عندما يزيد معدل خروجه من الجسم مع البول عن 300 مج من البروتين في اليوم وهو ما يسمى بالماكروألبومينوريا macroalbuminuria أما الكمية الأقل من ذلك فتسمى ميكروألبومينوريا microalbuminuria , ويوجد حاليا أنواع من الأشرطة تستطيع الكشف عنها وهو ذو أهمية حيث أن له دلالة بالنسبة لخطر إصابة الكلى عند مرضى السكر ويعتبر إفراز البروتين بالبول بمعدل 30 مج في اليوم لا معنى له insignificant من الناحية المرضية.

كما يجب عمل التحاليل التي تحدد كمية البروتين الموجودة بالبول وخاصة للمرضى المعرضين للإصابة بأمراض الكلى وخاصة مرضى السكر. كما أن نسبة البروتين إلى الكرياتنين protein-to-creatinine أو نسبة الألبومين إلى الكرياتنين albumin-to-creatinine ratio يمكن قياسها.

وفى حالة وجود نسبة عالية من البروتين بالبول يجب إعادة التحليل بعد أسبوعين , وفى حالة وجوده بنسبة عالية أيضا فإن ذلك يكون مؤشر لوجود فقدان مستمر للبروتين بالبول persistent proteinuria وعند ذلك يجب الاستمرار في عمل فحوص أخرى لتقييم وظائف الكلى كما يجرى تحاليل لتحديد نسبة اليوريا urea و الكرياتنين creatinine بالدم لمعرفة قدرة الكلى على التخلص من فضلات الجسم . وقد يحتاج الأطباء لأخذ عينة من نسيج الكلية renal biopsy .

علاج فقدان البروتين في البول

تعطى مستحضرات الإسترويد ومثبطات المناعة عندما يكون السبب هو متلازمة الكلى .

يجب ضبط مستوى السكر في الدم عند مرضى السكر ومتابعة قياسه و إتباع نظام غذائي صحي وممارسة قدر من الرياضة وأخذ العلاج , والمرضى الذين يعانون من السكر وارتفاع الضغط معا يصف لهم الأطباء في العادة أدوية تحفظ لهم الضغط عند مستوى اقل من 130 على 80 وهم يفضلون في أغلب الحالات إعطاؤهم المجموعة الدوائية التي تعمل من خلال تثبيط مادة الأنجيو تنسين ACE (angiotensin-converting enzyme) inhibitors أو إلغاء تأثيرها من خلال مجموعة دوائية مشابهة ARBs (angiotensin receptor blockers) وهذه المجموعة الدوائية علاوة على حفاظها على ضبط ضغط الدم فإنها تحافظ على الكلى وتمنع زيادة التلف بها .

أما مرضى ارتفاع ضغط الدم فقط ولا يعانون من مرض السكر أيضا فيصف الأطباء لهم الأدوية السابق ذكرها مع أدوية أخري مثل مدرات البول diuretics كما يجب حفظ الضغط عندهم عند معدل أقل من 130 عل 80 .

كما ينصح الأطباء في هذه الحالات بتقييد كمية ملح الطعام والبروتين التي يتناولها المريض ويمكن الاستعانة بمتخصص في التغذية للمساعدة في إتباع برنامج غذائي صحي في هذه الحالات .

Mesothelioma

============ Mesothelioma is a type of cancer that develops from the thin layer of tissue that covers many of the internal organs known as the mesothelium).[1] The most common area affected is the lining of the lungs and chest wall.[2][3] Less commonly the lining of the abdomen and rarely the sac surrounding the heart,[4] or the sac surrounding the testis may be affected.[5][2] Signs and symptoms of mesothelioma may include shortness of breath due to fluid around the lung, a swollen abdomen, chest wall pain, cough, feeling tired, and weight loss.[2] These symptoms typically come on slowly.[6] Greater than 80% of mesothelioma is caused by exposure to asbestos. The greater the exposure the greater the risk.[3] As of 2013 about 125 million people have been exposed to asbestos at work.[7] High rates of disease occur in people who mine asbestos, produce products from asbestos, work with asbestos products, live with asbestos workers, or work in buildings containing asbestos. Often it takes around 40 years for the disease to begin.[3] Washing the clothing of someone who worked with asbestos also increases the risk.[7] Other risk factors include genetics and infection with the simian virus 40.[3] The diagnosis may be suspected based on chest X-ray and CT scan findings, and is confirmed by either examining fluid produced by the cancer or by a tissue biopsy of the cancer.[6] Prevention centers around reducing exposure to asbestos.[8] Treatment often includes surgery, radiation therapy, and chemotherapy. A procedure known as pleurodesis, which involves using substances such as talc to scar together the pleura, may be used to prevent more fluid from building up around the lungs.[9] Chemotherapy often includes the medications cisplatin and pemetrexed.[6] The percentage of people that survive five years following diagnosis is on average 8% in the United States.[10] In 2013 about 50,000 people had mesothelioma and 34,000 died from the disease.[11][12] Rates of mesothelioma vary in different areas of the world. Rates are higher in Australia and Great Britain and lower in Japan.[3] It occurs in about 3,000 people per year in the United States.[13] It occurs more often in males than females. Rates of disease have increased since the 1950s. Diagnosis typically occurs after the age of 65 and most deaths occur around 70 years old.
